We are seeking a compassionate, experienced Clinical Nurse Practitioner to serve patients across Tarrant County in both hospice and palliative care settings. Key Responsibilities

As a Clinical NP, you will play a vital role in guiding patients and families through complex care journeys with dignity and compassion:

  • Partner with physicians and the interdisciplinary team to deliver personalized, high-quality hospice and palliative care
  • Provide in-home clinical visits, meeting patients where they are most comfortable
  • Educate and support patients and families on disease progression, comfort care, and next steps
  • Use your clinical expertise to assess, manage, and adjust care plans in real time
  • Perform face-to-face evaluations and document patient progress and needs
  • Collaborate closely with RN case managers and care teams to ensure continuity of care
  • Be a steady, compassionate presence during some of life’s most vulnerable moments

Qualifications

  • Licensed Nurse Practitioner (APN) in Texas
  • Active RN license in Texas
  • Minimum 2 years NP experience (hospice or palliative care strongly preferred)
  • Deep understanding of end-of-life care, pharmacology, and symptom management
  • Compassionate communicator who can support both patients and families
  • Comfortable working independently while collaborating with a care team
  • Reliable transportation and ability to travel throughout Tarrant County

Work Environment

Works in patients’ homes in various conditions; possible exposure to blood and bodily fluids and infectious diseases; the ability to work a flexible schedule; the ability to travel locally; some exposure to unpleasant weather; PRN emergency call.

  Physical and Mental Effort

Prolonged standing and walking are required, with the ability to lift up to 50 lbs and move patients. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient/family individualized psychosocial needs. Requires hand-eye coordination and manual dexterity.
